Can surgery prevent recurrent patellar dislocations better than conservative treatment?

Acute patellar dislocations can be debilitating and often require prompt intervention to restore function and stability to the knee joint. In 2009, a study conducted by Gilberto Luis Camanho, et. al., aimed to compare the outcomes of surgical and conservative treatments for this condition. The study analyzed patient data to evaluate factors influencing patellofemoral instability and assessed pain relief and overall quality of life. Here, we summarize the findings of this research.

The study involved 33 patients who had experienced acute patellar dislocations. These patients were divided into two groups: 16 received conservative treatment, which included immobilization followed by physiotherapy, while 17 underwent surgical intervention. To assess predisposing factors for patellofemoral instability, the researchers performed radiographic examinations. Additionally, the study employed the Kujala questionnaire to measure improvements in pain management and quality of life. Statistical analysis included the chi-squared test, t-test, and Fisher test, with a significance level set at P < 0.05.

Both treatment groups were comparable in terms of age and gender distribution. However, the conservative treatment group exhibited a higher incidence of recurrent patellar dislocations, with eight patients experiencing relapses. In contrast, the surgical treatment group showed no recurrences. Moreover, the surgical intervention group achieved a significantly better mean score on the Kujala test (92) compared to the conservative treatment group (69).

In conclusion, the study suggests that surgical intervention for acute patellar dislocations yields superior results compared to conservative treatment. Notably, the surgical approach demonstrated a complete absence of recurrent dislocations, while the conservative treatment group experienced eight recurrences. Furthermore, patients who underwent surgery reported a notably higher mean Kujala score, indicating improved pain management and quality of life. These findings emphasize the importance of considering surgical options for acute patellar dislocations to minimize the risk of recurrence and improve patient outcomes.
